QueensCare is opening the Nuziard Scholarship program to a limited number of applicants for the 2010-2011 academic year.Gene and Marilyn Nuziard Scholarship Fund
The Board of Directors of QueensCare has named its scholarship fund as a small tribute to Gene and Marilyn Nuziard for their 16 years of service to QueensCare. During 8 years as Chairman of the Board and through his ongoing service as a member of the board and many committees of the board, Mr. Nuziard has shared his knowledge and experience from the national business community to help QueensCare develop and expand the programs it offers today to the underserved of Los Angeles County.
Purpose
The purpose of this scholarship fund is to assist in alleviating the problem of the years long shortage of medical healthcare professionals, by providing funds for persons who wish to obtain an education to qualify them as providers of healthcare services and who do not have the financial means to pay for such an education.
The scholarships are available primarily for the education of assistants and technicians, nurses, physicians, licensed vocational nurses, registered nurses, nurse practitioners, and primary care physicians; and secondarily, for the education of physician specialists.
Amount
Scholarship recipients may be awarded up to $9000 per year for students in a 4 year university. Those attending professional or community colleges and part time students are eligible for smaller awards. The funds may be used for tuition, fees and books.
Eligibility
Students should be enrolled in one of the healthcare disciplines listed above. A minimum grade point average of 2.5 is required for community college and professional school students, 3.25 for undergraduates and 3.0 for graduate students. Funding is available to those whose family income is at or below 200% of the Federal Poverty Level and who express an interest in working with the underserved here in Los Angeles County upon graduation.
Application Process
Applicants are required to submit:
- A Gene & Marilyn Nuziard Scholarship & Loan Fund Application Form.
- A one page typed personal statement addressing the following:
- Achievements
- Academic Potential
- Career Goals
- Community and / or school involvement
- Scholarship and Student Loan Applicants are required to submit two “Recommendation Forms” with letters of recommendation attached. The letters of recommendation can be from any of the following: teacher, professor, guidance counselor, community leader, church leader, and / or immediate supervisor.
The individual (evaluator) should comment on your scholastic achievements and personal strengths such as enrollment, academic abilities and potential, maturity, motivation, self-confidence, leadership, perseverance, extra curricular activities, and community involvement. - A copy of the FAFSA or Student Aid Report.
